If you would like to make a submission for a formal quote from Midwest Builders’
Casualty we require that you provide the following information:
• Estimated annual payroll by NCCI Classification
• Loss history for the last three years including
a recent evaluation
produced by the current carrier
Additionally, we would ask that Brokers please submit an ACORD application with
risk identification number, if available, along with the most recent three years of both premium and loss data.
A preliminary quote can be quickly obtained using our Get A Quote function, however
this does not constitute a formal offer of insurance coverage and may differ from
